This service is offered as one in a suite of services and ministries (coaching, preaching, teaching, facilitation, consultation, and poetry) available from this provider. The purpose of this document is to (1) outline what coaching is about, and (2) summarize the commitments to which you and your Coach agree as part of the coaching relationship. Please let your Coach know if you have questions about this agreement. 

What is Coaching?

Coaching is a professional relationship between you (as “Client” or “Coachee”) and a professional Coach. You can think of it as a partnership in which your Coach provides a safe space and relationship in which you can reflect, share, and discuss things in life and work that are important to you. People often secure a coach to get unstuck, clarify goals, and to identify actions they can take to move forward in meeting those goals. Coaches honor each Client as being resourceful, capable, and the expert in their own life.

 

It is the Coach’s role to help you: 

Fundamental to a successful coaching experience is learning. At times this may involve new discoveries about the way your actions may impact situations you would like to improve. This type of self-discovery, although sometimes uncomfortable, may be essential to achieving the change you desire. 

The Coaching Agreement

This agreement between you and your Coach shall begin on

(date) for a total number of up to six (6) one-hour sessions of coaching. All coaching should be completed within six (6) months of the start date, unless otherwise agreed upon by the Client and Coach. Coaching sessions will be conducted via phone or ZOOM at scheduled times agreed upon by you and your Coach. Your initial coaching session is pro-bono, and includes time for reviewing the coaching agreement and addressing questions about the process. Subsequent coaching sessions shall be offered for an hourly rate per session payable to the Coach by Zelle or Cash App at least one week prior to the coaching session. Multiple sessions can be paid for in advance. 

Disclaimer

I understand that coaching is NOT therapy, nor is my Coach qualified to give legal, medical, or financial advice. I accept full responsibility for all actions I take or choose not to take as a result of coaching, and my Coach cannot be held responsible for any action taken or not taken as a result of my participation in coaching.

Further, I:


*Agree to be present and on time for all coaching conversations to which I commit.


*Realize that I am responsible for my own growth and learning.


*Understand there may be times in the coaching process when I feel some discomfort, especially when reflecting or engaging in new actions or behaviors as I work toward my goals.

 

*Promise that if I’m currently under the care of a mental health professional, I have consulted with him/her regarding working with a Coach and that this person is aware of my decision to proceed with the coaching relationship. 


*Acknowledge that information I share with my Coach will be held in confidence, within limits required by law (which would include disclosures of self-harm, harm to others, illegal activities, etc.). There may be instances in which information I share in coaching would be helpful to share with another professional entity. In these instances, my Coach will either ask for permission to share the information and/or encourage me to share directly. If I have questions about confidentiality, I will consult with my Coach.


*Understand I do not need to discuss anything I am not comfortable sharing.


*Acknowledge that coaching is a resource, and that it is up to me to take advantage of it.


*Recognize that coaching is a shared responsibility. I agree to communicate any concerns I have about the nature of my coaching relationship, my experience of the coaching relationship, or any particular interactions with the Coach. My Coach, who holds my well-being and success as their primary focus, will work with me to address my concerns.


*Agree to give at least 24-hours’ notice if I need to change or cancel a session. Similarly, I will let my Coach know if I am running late to a scheduled session. I understand that if I fail to provide 24- hours’ notice, my missed appointment may count as a completed session.


*Understand that if I’m more than fifteen (15) minutes late for a private session, my Coach may offer my appointment time to a different Client. 


*Understand that I can choose to discontinue coaching at any time.

 

*Acknowledge that my Coach may terminate coaching if they feel that it is no longer serving me, if they believe a different resource is required, and/or if I fail to adhere to the terms of this Agreement. 


*Agree that my Coach may keep basic records of my contact information, the hours and nature of our coaching to submit to obtain or maintain certification.
